The under eye area is one of the most common concerns I hear about in consultations. Patients describe looking permanently tired, older than they feel, or like they always have shadows under their eyes regardless of how much sleep they get. For many of them, under eye treatment specifically under eye treatment can make a significant difference. But it is also one of the most technically demanding facial areas to treat, and it's important to approach it carefully.

What causes the under eye "hollow" look?
The appearance of dark shadows and tired eyes is usually caused by volume loss in the under eye area the groove that runs from the inner corner of your eye diagonally towards the top of your cheek. As we age, fat pads in this area reduce and the skin thins, creating a shadow that gives the hollow, dark appearance most people are trying to address.
Importantly, not all dark circles have the same cause. Some are caused by pigmentation brown discolouration from sun damage or genetics. Others are caused by blood vessels showing through thin skin. These are distinct from the skin quality or structural concerns that may need different approaches. If your dark circles are primarily pigment related, under eye treatment may not be the right solution and I'll tell you that honestly during your consultation rather than proceeding with a treatment that won't deliver what you're hoping for.
What is under eye treatment?
Under eye treatment uses hyaluronic acid (HA) a naturally occurring substance in the body to restore volume in the under eye area. The filler is placed carefully to lift the shadow, smooth the transition between the lower eyelid and cheek, and refresh the overall appearance of the eye area.
Hyaluronic acid fillers are fully reversible using an enzyme called hyaluronidase if required, which is an important safety factor in an area as delicate as the under eye. Results typically last between 9 and 18 months, depending on the product used and your individual metabolism.
Why the assessment matters here more than anywhere
The under eye area is close to several important blood vessels, including the infraorbital artery. This is why it is critical that under eye treatment is performed by a clinician with prescribing authority, strong anatomical knowledge, and the ability to manage complications if they arise. As a registered nurse prescriber with over 15 years of NHS clinical experience, I approach this area with particular care.
Every patient I see for under eye treatment begins with a thorough consultation. I assess your anatomy the depth and position of your under eye area, the thickness of your skin, whether you have any relevant health history and I give you an honest answer about whether I think filler is the right approach and what I expect the outcome to look like. If I don't think it's right for you, I'll say so and explain my reasoning. I'd rather turn a booking down than deliver a result that doesn't serve the patient.
What does the procedure involve?
The treatment appointment typically lasts 30 to 45 minutes. Before injecting, I apply a topical anaesthetic cream to the area and wait for it to take effect which usually takes around 20 minutes. This makes the procedure comfortable for most patients.
I use either a fine needle or a cannula (a blunt tipped tube) depending on your anatomy and the technique best suited to your individual treatment. I work slowly and carefully, assessing the result throughout rather than injecting a fixed volume and hoping for the best.
After the procedure, there is typically some mild swelling and possibly a small amount of bruising. The under eye area area is prone to swelling initially, so the final result is usually better assessed two to four weeks after treatment rather than immediately. I always schedule a review appointment so we can assess the outcome properly.
What results can I expect?
Done well, under eye treatment creates a smoother, more rested appearance. The shadow is lifted, the hollow is softened, and the eye area looks refreshed rather than "treated." My approach is conservative I'd rather achieve a subtle, natural improvement and top up if needed, than over correct and leave you looking unnatural.
Individual results vary, and I'll always give you a realistic expectation during your consultation rather than overpromising. Some patients see significant improvement; others, particularly those with very thin skin or structural anatomy that makes treatment more complex, may see a more modest improvement or may not be suitable candidates at all.
